Our Care Coordination staff coordinates services from respite to extensive in home services and promote the health and well-being of over 3500 people statewide. Through EIM you will be supported in maintaining your independence while you remain in your own home.
The Care Coordination process is centered around you. We will work with you to develop the best possible support plan to help you remain at home. We will work with your families, loved ones, and health care professionals to identify your needs and preferences so we can link you to the best services available in your community.
